The record is shared. The platforms are not copies of each other.
Most networks run a claims system, a credentialing vendor, a clinic EMR and a monitoring portal, then reconcile them by interface engine and spreadsheet. eKlotho removes the reconciliation.
- The IPA platform (Nexus), the EMR, RPM and CCM read and write the same patient record — coverage status, care gaps and program data are the same rows, not copies kept in sync.
- Access is computed from the contracts between regulators, plans, IPAs, practices and providers, so a clinic and its IPA each see exactly their slice of the same chart.
- A patient can be in RPM and CCM at the same time: one enrollment per program, one time log that tells them apart, one monthly review.
- Lab Services and the SoCal dispatch platform are separate systems. They connect through provider and partner APIs, and the EMR accepts lab results through its result-ingest API — no interface engine in between.
